Permit Request Instructions

All of the red blocks are required. 


Step 1:  Decide if you are going to walk-in or if you need an appointment.

 

  • Monday, Tuesday, Thursday, Friday
    8:30AM until 11:00AM Walk-ins Only
    2:00PM until 4:00PM Appointments Only
  • Wednesday
    Noon until 4:00PM Appointments Only

Step 2:

 

Once you complete the form below:

If you're at the Police Department:

  • Fill out the form
  • Print the form (form prints in a secure location)
  • Close the form
  • Wait in line for assistance at the front window

 

     If you're not at the Police Department: 

  • Print, sign and date the completed request
  • Payment of $50 (check, money order, or credit)
  • Bring the request and your official government ID to:

No.  We do not accept permit applications via mail.
